Campers Insurance:
Our nurse requests that all youth attending Camp Life be in good health to avoid the spread of communicable diseases. Our insurance is a limited liability policy. All campers should have primary coverage by family group insurance policies for emergencies, accidents, and health-related problems.

Health Form

1. Download and Complete the Registration/Health Form
2. Parent or guardian must sign the form
3. Mail it to us at the address you'll see on the Registration page. If necessary (too late to mail), you may bring your signed medical form with you to the Camp.

Don't forget that you must also complete the registration form in order to register your camper.
